Patients placed on telehealth are provided with 4G tablets pre-loaded with HRS software. The tablets are paired with Bluetooth biometric devices, which allow patients to take their blood pressure, weight, heart rate, and more. The vital signs are automatically transmitted to the tablet and electronically delivered to the patient’s nurse, allowing clinical staff to quickly address any abnormalities or changes in medical status. The ability to remotely monitor patients allows clinicians to identify patients who are at risk for hospital readmissions or emergency department visits.

The telehealth software facilitates patient participation by providing daily medication reminders and disease specific educational content. Additionally, patients can actively communicate with their clinicians using the tablet’s text, phone call, and video chat features.

1st Care Home Health Services uses the HRS telehealth software to remotely monitor high risk patients and subsequently reduce readmissions, improve patient outcomes, and increase patient satisfaction. Telehealth patients are actively monitored by clinical staff who communicate patient updates to case managers and physicians.
