Landmark Health Extends Medical House Calls To More Patients

Landmark Connect reaches the most clinically vulnerable living in facilities

Huntington Beach, CA /PRNewswire-USNewswire/ - Landmark Health announces today their latest initiative to expand their home-based medical program to reach more people in need of high-touch care. Landmark Connect provides primary care to residents living in skilled nursing facilities (SNF), independent living facilities (ILF), assisted living facilities (ALF), continuing care retirement communities (CCRC), and other long-term care (LTC) facilities.

By treating people in place, Landmark Connect providers can focus on proactively managing residents' health conditions and ensuring personalized care coordination to help avoid unnecessary hospitalizations.

Under the Landmark Connect delivery model, patients residing in contracted care facilities, regardless of their insurance, can participate in the Landmark program. By embedding medical care teams in living facilities, Landmark is able to build relationships with patients, facility staff and families to improve early identification of changes in residents' status and monitor ongoing treatments. The result is better care, higher quality outcomes, and less unnecessary trips to the hospital.

Jennifer Gilchrist has been named President of Landmark Connect. Jennifer joined Landmark in January 2015 as Vice President, General Manager of the Albany, NY market. She successfully led the development and growth of that market, which recently hit a milestone patient engagement of more than 3,000 lives. In November 2016, Jennifer was promoted to Regional President of East Coast Operations where she leveraged her experience to benefit Landmark on a larger scale.
