The agreement calls for SCAH’s Integrated Chronic Care Model team to consult in the ongoing development of HealthWyse’s chronic care management solution.
SCAH’s Beth Hennessey and Paula Suter are award-winning experts who developed the Integrated Chronic Care Model. This collaboration is designed to further the goals of improving patients’ quality of life and delivering satisfaction to clinicians. This will be accomplished through comprehensive technology support for best practice at point of care.
