Care Coordination Company Doubles Efficiency With Home Care App

Some Apps Better Than Others At Promoting Well-Being

MobileSmith home Care App helps Integra Service Connect streamline its program.

Integra ServiceConnect leveraged a new native app created by MobileSmith for its community-based service coordination program, allowing it to streamline and optimize the program and double its efficiency. Built with the MobileSmith platform without coding, the iConnect app facilitates data entry and doubles work efficiency for the employees of Integra ServiceConnect.

Integra has spent nearly twenty years servicing communities in multiple states, deploying “feet-on-the-street” teams of non-clinical Community Coordinators who worked to find, engage, and connect underserved individuals to better healthcare outcomes by helping them overcome barriers to meeting their healthcare needs. Integra works with healthcare plans, providers, and government organizations.

And, as a recent Integra blog points out, reducing these costs is crucial, since clinical care determines only 20 percent of health outcomes. In fact, community-based services provided by trained, non-clinical Community Health Workers are a crucial part of the overall healthcare management process.

Available for iOS and Android and deployed internally via enterprise distribution, the iConnect app allows employees to:

  • Securely log in and get a daily list of patients to visit;
  • Pull up patient data by location and demographics; retrieve and edit data;
  • Enter notes as they speak with the patient (an estimated 50 percent drop in time spent on data entry and editing, compared to the previous process);
  • Identify and immediately address care gaps.

Working together with MobileSmith, Integra was able to develop the iConnect app for use by those employees, and the results have been dramatic. “Life is so much easier for our Community Coordinators with the iConnect app,” said Rob Bayne, Data Manager at Integra ServiceConnect. “Online data entry, that used to be cumbersome and redundant, now takes half as much time, and can be done on-the-fly.”
